Mistake #1 - The profile photo should convey who you are to those people looking at it. Too many profile photos are so abstract that it means nothing to anyone except to who posted it. If you are in a professional field, then don't post a photo of yourself taken when you were a baby. It means absolutely nothing to the viewer.

Mistake #2 - posting an arm's length hand-held photo you took of yourself dressed in a T-shirt against someone's fence. That leaves a great impression.

Mistake #3 - not having someone with some camera savvy shoot your photo. That will eliminate 90% of the potential problems.

You want your profile picture to tell a story about you.  You want it to scream with your personality and even share with your friends, family and potential clients, the things you love and feel passionate about.  If you're a business owner or service provider, express that in your profile picture.  So, I would say the first mistake is not having a profile picture at all, and the second, well....  maybe thinking a picture is just... a picture :)

If you're looking for a professional look, the first mistake would be to not hire someone who can give you professional results for your portrait.

A couple of big mistakes are (1) lighting - washed out, low contrast, extreme shadows - that makes seeing your face and its features difficult, and (2) taking it so far away that we can't tell who the photo is supposed to be showing..
